EMS Inspection Forms and Information

Effective April 1, 2021, IDPH will resume in-person transport vehicle inspections. These inspections will be scheduled in advance by an IDPH EMS Inspector.

Transport providers with vehicles due for inspection prior to April 1, 2021, should self-inspect their vehicles and send the completed inspections to the respective IDPH Emergency Medical Services (EMS) Inspector based on the regions below.

System Modifications requiring an inspection will still need to be reviewed and approved by the EMS Medical Director or EMS System Coordinator and the Regional EMS Coordinator before inspections can be entered into the licensing database.
